Mother Claire Flanagan had a history of social services involvement.

A probe into the death of a toddler killed by her mother’s violent lover uncovered a catalogue of failings by child protection agencies.

Violet Mullen, 15 months, endured weeks of assaults by Gary Alcock before she died at home in Oldham in January 2010.

Her injuries resembled those from a car crash.

A serious case review made 90 recommendations and highlighted a string of mistakes by 11 agencies.

It described how both Alcock and the mother, Claire Flanagan, were known to social services. But weeks before Violet died, social workers visited the home and failed to ask what Alcock, who had a conviction for domestic abuse, was doing there.

The probation service, First Choice Homes, family courts service and health bodies were also criticised.

Problems included communication and a lack of understanding of domestic abuse.

The report said the death could not have been predicted or prevented. Alcock was jailed for life for murder. Flanagan, who turned a blind eye, was jailed for five years.
